Palliative care is specialized medical care geared towards managing and addressing the physical and psychosocial symptoms associated with serious illness. At Affinity Palliative Care, our services include in-home visits scheduled once a month by a nurse practitioner to address physical symptoms caused by the disease or the course of treatment such as: nausea, fatigue, anxiety, insomnia, loss of appetite, pain, depression, shortness of breath, edema, and constipation. Additionally, we can offer more visits to manage any developing situation during the course of illness.

Home Based Palliative Care:
While much of Palliative Care services are provided in a hospital or clinic setting, Affinity offers home-based care. Within this program, we are able to meet patients at their home (residence, assisted living facility, skilled nursing home, etc.) and provide them with Palliative Care. Our Nurse Practitioner completes a comprehensive exam, patient medical and social history, and individualizes a plan/goals of care.
Our Palliative Care program aims to prevent unnecessary hospitalizations for issues such as dehydration, UTI, or shortness of breath by managing these conditions at home. Patients benefit greatly as their symptoms are managed quickly and in comfortable surroundings. This in turn translates into fewer hospital admissions and reduced costs for the patient/family as well as the overall healthcare system. Ultimately, we hope to improve the quality of life for patients suffering from chronic or terminal diseases by taking a proactive approach with their care as they progress in their illness.

Advanced Care Planning/Post:
Advanced Care Planning (commonly, ACP) refers to a discussion of the type of care (medical intervention) a patient would want to receive if they were to become unable to speak for themselves, including a discussion of advance directives. These wishes and preferences can be modified or discarded throughout the disease process as circumstances change.
Our Palliative Care team can speak with the patient, family, and primary caregiver about disease progression and treatment options. These ongoing conversations aim at guiding the healthcare providers as well as the patient's family and caregivers in an effort to respect the patient's wishes regarding their care.
For our patients in Georgia under Palliative Care, we ask patients to voluntarily complete the Georgia POLST (Physician Orders for Life-Sustaining Treatment) form as developed and recognized by the Georgia Department of Public Health. Our Social Worker and Nurse Practitioner can help provide a well-rounded understanding of the patient's options and how to fill out a POLST form.
