What is

Palliative Care?

Palliative care is specialized medical care geared towards managing and addressing the physical and psychosocial symptoms associated with serious illness. At Affinity Palliative Care, our services include in-home visits scheduled once a month by a nurse practitioner to address physical symptoms caused by the disease or the course of treatment such as: nausea, fatigue, anxiety, insomnia, loss of appetite, pain, depression, shortness of breath, edema, and constipation. Additionally, we can offer more visits to manage any developing situation during the course of illness.

Our team provides a unique Palliative Care Service:
We come to you

  • We provide a staff of experienced clinical     professionals
  • We have a community-based large service area
  • We coordinate care services with other healthcare providers such as diagnostics, infusion and home sleep studies.
  • We promote meaningful transitions with disease progression: hospice & home health

What palliative care services are offered by Affinity Hospice?

If you or a loved one is facing a serious or terminal illness, you may be feeling anxious or isolated. At Affinity, we recognize that chronic illness is taxing on both the patient and the caregiver. While traditional medicine can provide basic care, it is often focused on the disease process and not the patient as a whole. We believe that Palliative Medicine with emphasis on quality of life is a standard of care that every patient deserves. At Affinity Palliative Care, we treat each and every one of our patients as if they were members of our own family.

While some palliative care outpatient clinics provide the basic services necessary to manage the patient's care, our family-owned program aims to provide support and comfort to both the patient and their loved ones in the familiar setting of their own home. Our Palliative Care program provides:

A Home Based Palliative Care

A Advanced Care Planning

Home Based Palliative Care:

While much of Palliative Care services are provided in a hospital or clinic setting, Affinity offers home-based care. Within this program, we are able to meet patients at their home (residence, assisted living facility, skilled nursing home, etc.) and provide them with Palliative Care. Our Nurse Practitioner completes a comprehensive exam, patient medical and social history, and individualizes a plan/goals of care.

Our Palliative Care program aims to prevent unnecessary hospitalizations for issues such as dehydration, UTI, or shortness of breath by managing these conditions at home. Patients benefit greatly as their symptoms are managed quickly and in comfortable surroundings. This in turn translates into fewer hospital admissions and reduced costs for the patient/family as well as the overall healthcare system. Ultimately, we hope to improve the quality of life for patients suffering from chronic or terminal diseases by taking a proactive approach with their care as they progress in their illness.

Advanced Care Planning/Post:

Advanced Care Planning (commonly, ACP) refers to a discussion of the type of care (medical intervention) a patient would want to receive if they were to become unable to speak for themselves, including a discussion of advance directives. These wishes and preferences can be modified or discarded throughout the disease process as circumstances change.

Our Palliative Care team can speak with the patient, family, and primary caregiver about disease progression and treatment options. These ongoing conversations aim at guiding the healthcare providers as well as the patient's family and caregivers in an effort to respect the patient's wishes regarding their care.

For our patients in Georgia under Palliative Care, we ask patients to voluntarily complete the Georgia POLST (Physician Orders for Life-Sustaining Treatment) form as developed and recognized by the Georgia Department of Public Health. Our Social Worker and Nurse Practitioner can help provide a well-rounded understanding of the patient's options and how to fill out a POLST form.

Very well ran company. Will treat you right in what is usually a tough time.

We cannot say enough about this company! I believe they're the only one that offers 24 hour continuous care by nurses. We did have a personality conflict with one of the nurses which was promptly handled to our complete satisfaction very professionally. They not only cared for my father in law but the rest of us as well. This is a situation that is tough on all of us. All we want is for our loved one to transition peacefully without pain. The staff not only met but exceeded all of our wishes. Please call on them, you will not be disappointed.

This hospice is fabulous. They come out however many days I want. If I have emergency or just need someone the on call nurse comes out. They are very dependable. They are genuinely good and want to help. I am so glad my doctor referred them. Just a blessing.

Amazing group of caring professionals who are passionate about what they do! ❤️

I don't normally leave online reviews, but this experience warranted one. We just lost our son to brain cancer and affinity Marietta was our hospice provider. The team we had were the best. They were kind, understanding, and very caring for our son and us. These people deserve special recognition, Leslie, our r.n., Samantha, our social worker, and her husband Devon, and Joy, the nurse that came out the day of his death. Y'all are truly angels in disguise.

A typical day at work with Affinity Hospices for me was meeting different client and meeting their needs of care . I learn that do your best no matter who the client is and show the client dignity and respect. the hardest part of my job is letting myself get to attach to a client and then they die. the most enjoyable part of my job is working with a great team and wonderful clients and family.
